Job Description

Requirements:

1. Active RN license in good standing, with eligibility to practice in multiple states.

2. Minimum of 3 years of clinical nursing experience.

6. Ability to work independently with minimal supervision.

Responsibilities:

1. Conduct telephonic assessments and provide clinical guidance to patients.

2. Educate patients and caregivers on disease management, medication adherence, and lifestyle modifications.

3. Coordinate care with other healthcare providers and community resources.

4. Document all patient interactions accurately in the EHR system.

5. Monitor patient progress and adjust care plans as necessary.

6. Participate in ongoing professional development and training programs.

7. Adhere to all regulatory compliance and quality assurance standards.

Educational Qualifications:

1. Bachelor of Science in Nursing (BSN) preferred, but an Associate Degree in Nursing (ADN) with relevant experience will be considered.

Experience:

1. Minimum of 3 years of clinical nursing experience in a hospital, clinic, or telehealth setting.

2. Experience in case management or chronic disease management is a plus.
