As a leading provider of Double Qualified Special Needs Plan (DSNP) services, we're dedicated to empowering individuals, families, and communities to achieve optimal health and well-being. **Job Summary** As a Case Management Specialist, you'll be responsible for providing high-quality, patient-centered care to individuals enrolled in our DSNP program. You'll work closely with healthcare providers, community organizations, and other stakeholders to develop and implement comprehensive care plans that address the unique needs of each individual.
